The purpose of this form is to provide an efficient way for a member to request to be
tested in order to advance in Skill Level Rating. Please provide the information below to
be scheduled for a Rating Clinic.

By submitting this form, you confirm that you have reviewed both of the SKILLS GUIDES and RATING POLICY posted on the Club website under “Player Development” and that you are eligible and ready to test at the next available Rating Clinic:

your request will be reviewed by the Rating Committee and a response will be sent to you within 5 days.
